from what I could find there's no "normal" way to get fortune IV or efficiency IV. I admit that I only tried in single player but while using creative there was no way to get these using vanilla 1.5.2 so I suspect you're either using a pre-built which kind of give you IV efficiency or any other mod that changes the rank. I have no clue how you managed to get the item data value but when I spawned it in singleplayer it gave me that item only then with level III enchantment overall I have to say it's a very weird case.

Just out of sheer curiousity what version were you using the current 1.5.2. or a pre-release version? and any clarification/idea on how you got it just using books or an enchantment table?

you can get those enchantments back pretty easily actually. just buy a bunch of in game money and then craft the tools again and then buy the exp bottles and the enchanted books for what you need and make the tools enchanted with an anvil.
